Pre-surgical Clearances, Forms, etc.

The OMC physicians will not complete forms or provide clearances for surgical procedures, since this is one of the prime responsibilities of the personal physician.

OMC physicians will also not complete examination forms for insurers, Social Security, other employers or treatment facilities. Again, such certifications are the responsibility of the personal physician.
