About the Program
Mental Health Intensive Case Management Program (MHICM) is a Department of Veterans Affairs clinical program whose purpose is to help veterans who have had a history of mental illness to maintain themselves in the community to the best of their ability. Instead of relying on inpatient and emergency room care, we help veterans improve their lives and help them utilize outpatient, community and clinic-based services. The program is characterized by frequent (at least weekly) face-to-face visits and a variety of support services which utilize various VA and community services.
MHICM Services
- intensive case management in the home or place or residence
- coordination of primary medical & mental health care
- assistance with medication management
- crisis management
- information, referral and assistance in applying for needed benefits
- counseling regarding family issues, finances, housing alternatives, social skills, and activities of daily living
- access to vocational assessment and counseling
Who uses MHICM?
The program is targeted at veterans who have a history of mental illness and have a history of long and/or frequent psychiatric hospitalizations.
How does MCHIM Work?
Veterans must be eligible for VA services and enrolled in the New York Harbor Healtcare System. Veterans are referred through VA health care professionals, inpatient wards and outside resources.
Areas Served by MHICM
The MHICM team can serve veterans who reside in Manhattan, Manhattan, Queens and Staten Island.
