  1. Select the applicable license - either the standard U.S. license or the non-U.S. research-only license. Use the PDF version to enter data electronically. The License requires a signature. Please print and send via postal service or fax, or submit signed document via e-mail.

    1. License Agreement to Lease NLM Databases in Machine-Readable Form: pdf, html
This is the standard license agreement for U.S. organizations only. The following criteria apply: the organization is incorporated in the U.S.; the signing official is a principle in the organization who is located in the U.S. and is not simply an authorized agent; and any financial payment to the NLM is issued from an U.S. account (although there are no fees for leasing NLM databases in the foreseeable future).

    2. License Agreement for Non-U.S. Organizations for Use of MEDLINE/PubMed and Other NLM Databases Solely for Research Purposes: pdf, html
This is the license for non-U.S. organizations using NLM data for internal research projects only. No commercial citation search service may be offered.

  2. Complete the Intended Use Worksheet: pdf, html
The Intended Use Worksheet does not require a signature. Complete and send via postal service or fax, or attach document in e-mail. The License will be associated with the Intended Use Worksheet after both are received at NLM.
