HIV-1/AIDS and STD among adolescents in Mexico.

OBJECTIVE: To analyse epidemiological characteristics of HIV-1 and other STD infection among adolescents in Mexico. METHODS: Analysis of data from the National Registry of AIDS and HIV-1 and STDs National Sentinel Surveillance System. Laboratory: Blood samples were taken to process: HIV-1 (ELISA and WB), T. pallidum (VDRL and FTAbs), antiHerpes IgM (ELISA) and antiHBs (ELISA) and urethral or cervical samples for processing N. gonorrhoeae (Ag, ELISA) and C. trachomatis (Ag, ELISA). RESULTS: Up to January, 1992, 9239 AIDS cases have been notified, of these 205 (2%) are adolescents (11 to 19 yrs). Male: 160 (78%), m:f ratio 4:1, 35% homosexual, 16% bisexual, 11% heterosexual, 13% transfusion recipients, 23% hemophiliacs, 2% IVDU. Female: 45, 49% transfusion recipients, 47% heterosexual, 4% IVDU. Initial clinical manifestations: 7.3% PCP, 1.5% SK, 41.5% wasting syndrome, 5.4% neurological problems, 84.9% other opportunistic infections. HIV-1 and STD sentinel surveillance has been conducted among 2905 men with homosexual and bisexual practices and 5301 female prostitutes: 502 (17%) are male adolescents (M) and 585 (11%) female adolescents (F). Sociodemographic characteristics: Age 17.3 (M), 18.1 (F), schooling (elementary or less) 21.1% (M), 71.5% (F), single 93.8% (M), 76.1% (F). Sexual practices: No. of sex pts during previous 6 ms 10 (M), 189 (F), usage of condom during previous 6 ms 36% (M), 62% (F). Previous STD: 42.7% (M), 38.2% (F). IVDU: 0.2% (M), 1.2% (F). Genital examination: Genital ulcers 6.4% (M), 2.9% (F), genital-anal warts 11.6% (M), 3.9% (F). HIV-1 and STDs: HIV-1: 15.9% (M), 0.0 (F), T. pallidum 23.4% (M), 17.3% (F), N. gonorrhoeae 3% (M), 23.5% (F), C. trachomatis 2.9% (M), 19.3% (F), AntiHBs 4.7% (M), 5.9% (F), antiHerpes IgM 8.3% (M), 14.4% (F). Risk factors and HIV-1 STD interrelation: Risk factors that were consistently observed with infection of HIV-1 and other STDs were less usage of condom, no. of sexual pts. Association was observed between genital warts and HIV-1 among M. CONCLUSIONS: Main risk for AIDS among M are homosexual and bisexual practices. Sentinel studies among M with homosexual and bisexual practices show that usage of condom is not frequent. Prevalence of HIV-1 and T. pallidum is high. Risk of heterosexual transmission among F with AIDS has a greater relevance than among adult females. Although referred usage of condom is high among F prostitutes, prevalence of STD is high and indicates risk of spread of HIV, although at present, rate of HIV infection is low.
