HIV/AIDS continuum of care: treatment tools and advocacy for access in the resource-poor setting of Ukraine.

Issues: By elaborating an integral model of HIV/AIDS continuum of care, MSF included all components where gaps in care exist, by using Ministry of Health (MoH) structures and adding necessary components, improving accessibility and affordability of HIV/AIDS care. Description: Using prevention of mother-to-child transmission (PMTCT) as an entry point, MSF has been developing a continuum of care model for People Living with HIV/AIDS (PLWHA) in the southern regions of Ukraine since 2001; the program includes provision of affordable and replicable medical care within the project area. Services: Introduction of cost effective treatment protocols; HAART (130-200 patients from MSF PMTCT cohort); Drugs and prophylaxis for opportunistic infections; PCP prophylaxis to 800 mother and child; Diagnostic laboratory capabilities including CD4 count; Nevirapine according to a modified HIVNET012 protocol: Provision of Infant formula; Voluntary Counseling and Testing; Peer-support to mothers; Training of health professionals and PLWHA; Dissemination of IEC materials to the general public; Advocacy: Access to essential drugs and treatment; Empowering PLWHA; Solidarity campaign Lessons Learned: The delivery of care and treatment to PLWHA in Ukraine requires full and transparent cooperation amongst all Government bodies, NGOs and patient action groups. It is only when this occurs will the available resources be utilized in the most effective and efficient manner. In a continuum of care program, psychosocial support to patients by peer counsellors also plays a vital role in treatment adherence and the successful scaling up of the HAART. Recommendation: This model recommends that an effective co-ordination mechanism in the national and regional level with the obligatory component of Monitoring and Evaluation be developed and provision of resource allocation in Ukrainian Health care system be increased and followed by greater cooperation in the formulation of Action Plans and Strategies.
