Background: Jamaica and Trinidad are endemic locales for HTLV-1, the etiologic agent of HTLV-I-associated myelopathy/ tropical spastic paraparesis (HAM/TSP). Objective: To quantify the incidence of HAM/TSP in adults of African descent in Jamaica and Trinidad. Methods: Incidence rates for HAM/TSP were calculated from all incident cases in Jamaica (n=152) and Trinidad (n=35) during 1990-1994. Combined incidence rates for Jamaica and Trinidad were computed by weighting to population size. Results: The annualized incidence of HAM/TSP per 100,000 pop. in Jamaica and Trinidad was 2.8 and 3.2, respectively. Among the combined populations, incidence increased with age among both males and females, with a steeper increase among females. This resulted in an annualized incidence among females of 3.1 compared to an incidence of 1.6 among males. Conclusions: The overall incidence rates of HAM/TSP in Jamaica and Trinidad are similar to the rate reported for Japan. The higher incidence among females may reflect the importance of sexually acquired HTLV-I infection in HAM/TSP pathogenesis.
