FCC Report 43-04 General Instructions
FCC Report 43-04, the ARMIS Access Report, lists the jurisdictional separations and access charge data, which must be provided annually for each study area. It is comprised of the following table:

When errata occur, carriers must include in the transmittal letter a brief statement indicating the reason for the errata. Other explanatory notes must be included in the footnote section of the filing.
