The Hawai‘i Department of Health partners with AIDS Service Organizations in the community to provide case management and supportive services to people living with HIV as well as their spouses, partners and families. In order to access HIV supportive services, such services funded in Hawai‘i through the Ryan White CARE Act, an individual must enroll in a case management program. Case managers provide:

  • Medical Case Management to help individuals living with HIV to access primary medical care and removing barriers to remaining in treatment. Activities include assistance and support with:
    • Applying for entitlement program benefits such as Medicare, Medicaid, Veteran's Administration
    • Accessing HIV medical management services: HSPAMM, HDAP and H-COBRA
    • Accessing health-related services such as nutritional assessment & counseling, dental care, mental health services, substance abuse treatment, and home health services
    • Accessing support for transportation, food & housing
  • Specialized Case Management for individuals who have substance abuse and/or mental health issues and individuals who are homeless
  • Specialized Case Management that is culturally sensitive and culturally appropriate for Native Hawaiians
  • Specialized Case Management for Women, Children & Families
  • Social Services Case Management including assessment, counseling, referral, advocacy and follow up on a range of social service issues
  • Client Support Services including food bank, dental care, housing assistance, and mental health counseling.
