HIV/AIDS Epidemiology in Hawai‘i

As of December 31, 2007, a total of 3,011 AIDS cases were reported by health providers and 1,752 (58.2%) of these individuals are known to be deceased. AIDS incidence and mortality have declined in Hawai‘i and at the national level with the widespread use of the highly active anti-retroviral therapy (HAART) since 1996. Hawai‘i's AIDS case report rate for 2007 was 6.4 (per 100,000 population).

Although the majority of Hawai‘i's AIDS cases (92%) are men, the proportion of cases diagnosed in women has been increasing over time. Men who have sex with men (MSM) account for the majority of AIDS cases (73%), followed by injection drug use (IDU) (8%), MSM/IDU combination (7%), and heterosexual contact (6%). The proportion of AIDS cases related to MSM and IDU have decreased over time, and slightly increased for MSM/IDU and for heterosexual contact over time.

The majority of Hawai‘i's AIDS cases (59%) are Caucasians, followed by Asians (15%) and Hawaiians/Pacific Islanders (13%). Hawaiians/Part-Hawaiians account for 11% of AIDS cases, Hispanic 6%, and African American 4%. Over time the proportion of AIDS for Hispanics has been increasing, while the number and proportion of AIDS cases for Caucasians have been decreasing.

Most AIDS cases have been diagnosed in individuals in their thirties and forties. Over time there has been a decrease in the proportion of cases diagnosed in their twenties and an increase in the proportion aged over thirty. Pediatric cases have been less than one percent (17/2,011) of the cumulative diagnosed AIDS cases in Hawai‘i. The majority of the AIDS cases (73%) have been diagnosed in Honolulu County, followed by Hawai‘i County (13%), Maui County (10%), and Kauai County (4%).

As of December 31, 2007, the number of people living with AIDS (PLWA) was 1,259. PLWA are primarily males (89%), Caucasians (56%), and live in Honolulu County (68%). This number of PLWA represents only individuals diagnosed in Hawai‘i. It does not contain data on those diagnosed out-of-state but receive care services in Hawai‘i. It also does not include those who were diagnosed in Hawai‘i but then moved to another state.
