• Data Disk: Includes a choice of any or all of these preferences: license number, county, phone number, primary specialty, date of birth, date first licensed, expiration date, standing, license status, etc. For a full listing of available data fields, see the Data Request Form. License name, address, city, state, and zip code are standard. The disk is done in ASCII fixed-width, ASCII delimited, or CSV format on an ISO-9660 CD-ROM. A Data Request Form must be completed.  
• Malpractice Report: This database compiled from Negligence Claim Reports filed by insurers of professional liability policies under rules promulgated by the Insurance Division of the Oregon Department Of Consumer and Business Services. • Label Formatted Disk: This is a disk with names and mailing addresses only. It is formatted for six line labels, although can be requested as a CSV (Comma-Separated Value) suitable for mail-merge with popular desktop software or use by spreadsheets and desktop databases. STATUS

• Active: The licensee practices within the state.
• Inactive: The licensee practices and lives outside the state.
• Emeritus: The licensee is retired from active practice, but may do volunteer work without pay in Oregon.
• Locum Tenens: The licensee practices intermittently within the state, but resides outside the state.
• Limited License: Licensees who have limited licenses are restricted according to the particular type of limited license they have been issued. For a full explanation of all the various types of limited licenses see Limited Licenses Definitions.

• DPM: Podiatrist
