If you have a question about the license renewal process, please review the responses to the Frequently Asked Questions (FAQs).  These are responses to questions Board staff receive every renewal period, so it is likely your question will be answered by reading the FAQs.
 
 

Mailing Dates License Renewals
Mailing Dates of License Renewal Forms
 
The renewal forms for physicians (MD/DO), podiatric physicians (DPM) and physician assistants (PA) are being mailed out in ZIP code order over three weeks to spread out the return of these renewal forms to the Board.  The renewal form mail schedule is as follows:
  • 9/20/07 MD/DO and PA
  • 9/24/07 MD/DO
  • 10/1/07 MD/DO and DPM

If you have not received your license renewal form by 10/22/07, please complete a renewal form available to you on the Board’s web site, sign it, and send it in to the Board with the appropriate fee or your credit card information. The license renewal form on our web site will not have your information preprinted on the form. Be sure to complete this form completely to insure timely processing of your license renewal. We do not send renewal forms via facsimile (FAX) machine.

License Renewal Status Search Service  
 
You, and the hospitals and clinics where you practice, may check your license renewal status of the status of up to 25 licensees per inquiry.  The search results state whether the renewal form has been received by the Board, is being processed, or has been renewed (process is complete), and the date the Certificate of Registration was printed. 
 
The License Renewal Status search service will greatly assist you and the credentialing staff of hospitals, surgical centers and clinics and answer the question of whether the Board has received the license renewal form and whether a Certificate of Registration has been printed.   There is a link between the License Renewal Status and Verification of Licensure, so if a license has been renewed for the 2008-2009 biennium, credentialing staff can print a Verification of Licensure.
 
 

Reactivation

If you wish to return to Oregon to practice or wish to practice under a status where you can receive remuneration for your services, you are required to reactivate your Inactive, Locum Tenens, Active – Military/Public Health, Active – Teleradiology, Active – Telemonitoring, Emeritus, or Emeritus Inactive status to an Active, Locum Tenens, or Emeritus Active status.  
 
Please note:  The Reactivation process is a separate process from the License Renewal process.  You must still renew your Oregon license if you are going through the reactivation process, or your license will lapse at midnight December 31, 2007.  This will halt the reactivation process for your Oregon license.
