Subject: FUEL POOL CRANE FAILURE                                         

Discussion:

Millstone Unit 1 is permanently shut down and has initiated the          
decommissioning process.  On Monday, October 11, the licensee was moving 
a new fuel assembly out of the spent fuel pool, where it was stored, and 
into the new fuel storage vault.  During this move, however, the reactor 
building crane auxiliary hook continued to drift downward past the stop  
point and the fuel assembly came in contact with the refuel floor for a  
short period of time.  All fuel movements with the crane have been halted
pending the licensee's determination of the cause of the crane failure,  
and completion of appropriate corrective measures.  Prior to the crane   
problem and subsequent placement of the affected assembly into the       
storage vault, the licensee had succesfully transferred 16 assemblies    
from the spent fuel pool to the new fuel vault.  There was no visible    
damage to the new assembly and no radiological consequences from this    
event.  NRC informed the State of Connecticut                            

Regional Action:

The resident inspector is following licensee actions to investigate the  
crane problem.

Subject: LOSS OF NORMAL FEED TO SURRY UNIT 1 H AND UNIT 2 J EMERGENCY    
         BUSSES                                                          


Discussion:

On October 9, 1999, at 12:41 pm., the cabling between the C reserve      
station service transformer (RSST) buss work and the emergency switchgear
experienced an internal fault and caused a transformer lockout.  The C   
RSST is the normal feed for the Unit 1 H and Unit 2 J emergency busses.  
The #1 and #3 emergency diesel generators (EDGs) started on undervoltage 
to supply the Unit 1 H and the Unit 2 J busses respectively.  Prior to   
the event both Units 1 and 2 were at 100 percent power.  There were no   
unexpected effects on Unit 1; however, Unit 2 power decreased to         
approximately 96 percent due to the momentary power lost to a turbine    
electro-hydraulic control panel before the #3 EDG re-energized the J     
buss.  Unit 2 was restored to 100 percent power approximately two hours  
later.  Both units continued full power operation with the #1 and #3 EDGs
powering their associated emergency busses until approximately noon on   
October 11 when the C RSST was restored to full service.                 
                                                                         
The cable fault was between the cable conductor and the cable aluminum   
shielding at the point where the cable is attached to a lug.  The        
licensee suspects that a breakdown of the insulation and protective      
covering at this location occurred.  Samples of the material have been   
sent offsite for evaluation.  While the C RSST was out of service, the   
material at all twelve connection points was replaced.
