VENDOR: Foxboro PT21 FILE NO: 99-39-0

SOURCE DOCUMENT: EN 36122 REVIEWER: REXB, V. Hodge

NEW ISSUE. The vendor, Foxboro, reports a potential failure of contact
output isolator cards, N-2A0-L2C-R or N-2A0-L2C-R with ECEP 10273, that
may contain L0152CK relays manufactured by Communications Instruments
Inc. The subject relays include repair residual material on the contact
armature plate. As a result, the contacts on the subject relays may not
return to the normal de-energized position as designed. This defect is
limited to relays with a manufacturer's date code in the range 9834 to
9933.

The vendor is notifying all nuclear utilities that have purchased
affected relays, recommending replacement of such relays with ones that
the vendor will provide.
