NEW ISSUE. The vendor, Integrated Resources, reports failure of five
square root converters. The Dresden licensee had sent nine of these
converters to the vendor for testing and five of them failed. The vendor
sent them to the manufacturer, Barker Microfarads, for further analysis.
These items are aluminum electrolytic capacitor devices, Sprague Electric
Model TE1302 of manufacture date code 9322H.
The vendor reports that four of these converters are in safety-related
use, two at Nine Mile Point Unit 1 and two at Fitzpatrick and about one
hundred of them are in nonsafety-related use at the Dresden and Quad
Cities stations. The vendor notified all its affected customers.
On 09 Feb 99, the vendor confirmed by telephone that the cause of these
failures is a malfunction of the oversized end seal. At the time of
manufacture, the end seal was made larger than would be the case today.
The seal problem allowed the loss of moisture from the capacitor, causing
the equivalent series resistance to increase, meaning more production of
heat and further drying. The anode lead became unbonded from the
capacitor plate material to shorten the nominal 10 year shelf life of the
device.

NEW ISSUE. The vendor, Swagelok, reports a possible material defect in
the pipe fitting tee, part number SS-6-T and product identification
number R27GE0123B. The defect is a crack in the center of the forging.
The vendor supplied 20 of these tees to the Public Service Electric & Gas
licensee and 3 of them to the South Carolina Electric & Gas licensee. The
vendor is continuing its analysis.

NEW ISSUE. The vendor, Engine Systems, reports degradation of three
Synchrostart Model ESSB-4AT speed switches at the Hope Creek nuclear
power plant. These switches are standard components in the vendor's
safety-related EMD diesel generator control systems. Failure of the
speed switch could disable generator field flashing, resulting in failure
of the diesel generator to build output voltage. All three degradations
occurred because an incorrect resistor was installed on the power supply
card, resulting in heat-related damage to the other components on the
power supply card. The vendor is notifying all its nuclear customers.

Subject: UNANTICIPATED CONTRACT WORKER EXPOSURE FROM RADIOACTIVE PARTICLE
Discussion:
On February 12, 1999, the Crystal River Plant reported the discovery of a
radioactive particle in the lining of a worker's coat. The coat was
being worn by a contractor of the French firm Mecatiss. The particle was
detected in the coat during initial processing and prior to entry into
the licensee's radiologically controlled area (RCA). The licensee
recovered the radioactive particle which contained 0.0753 microcuries of
Cobalt-60. Based on assumptions regarding use of the coat, area of
exposure and shielding provided by the coat lining and worker's shirt,
the licensee estimated 1999 maximum doses to a 32 square centimeter area
in the vicinity of the worker's shoulder to be approximately five
millirem deep dose equivalent and eight millirem shallow dose equivalent.
No other radioactive particles were found on the individual, other
Mecatiss workers or in the protected areas visited by the workers. The
precise source of the radioactive particle has not been determined at
this time.
Regional Action:
Region II notified the State of Florida, NRC Office of International
Programs, Office of Nuclear Material Safeguards, and the Office of the
Executive Director for Operations regarding this incident. The licensee
has notified the State of Florida, Department of Health and is preparing
to notify the corporate office of the employee.
The NRC RII office was initially notified of this incident by the
licensee on February 12, 1999.

On February 16, 1999, Mr. John Sampson, D.C. Cook Site Vice President,
was named as American Electric Power Indiana/Michigan State President,
effective March 1, 1999. He succeeds Mr. Dick Boyle, who has been named
Vice President - Regulatory Services. No replacement for Mr. Sampson has
been named.

Subject: BYRON STATION SENIOR MANAGEMENT CHANGES
Discussion:
On 2/16/99, Mr. Ken Graesser, Byron Site Vice President, announced plans
for retirement. Mr. William Levis, currently Station Manager will be
prompted to Site Vice President. Mr. Richard Lopriore will join the
company on 3/15/99 as the Station Manager.
Mr. Graesser will continue with ComEd until July, 1999, where he will
lead development of a long range material condition plan for the Nuclear
Generation Group.
Mr. Lopriore is coming to Byron station from Ontario Hydro where he was
Assistant Vice President for Pickering Station Units 5 through 8. He also
held previous positions with Carolina Power and Light's Brunswick Nuclear
Power Plant and Vermont Yankee Nuclear Plant.
