NRC Information Notice 2000-20: Potential Loss of Redundant Safety-      
Related Equipment Because of the Lack of High-Energy Line Break Barriers 
Dated December 11, 2000                                                  
                                                                         
The U.S. Nuclear Regulatory Commission issue this information notice to  
alert addressees to the discovery of several high-energy line break      
concerns about redundant safety-related equipment at D. C. Cook Nuclear

Subject: Fuel Assembly Leakage at Sequoyah                               

Discussion:

The Tennessee Valley Authority (TVA) determined, during the Sequoyah Unit
2 October 2000 refueling outage, that one fuel rod in one of four lead   
test assemblies (LTAs) containing fuel pellets made with blended-down    
highly enriched uranium (HEU) had been leaking during the last operating 
cycle. The LTAs were manufactured by Framatome Cogema Fuels (FCF) as part
of a Department of Energy (DOE)-sponsored program to use HEU as fuel in  
one or more of the TVA nuclear reactors. There are 264 fuel rods in each 
of the four LTAs, or 1056 rods total.                                    
                                                                         
The LTAs had been in the core since the last refueling outage in April   
1999 and had burnup of approximately 24,000 mwd/mtu. TVA discovered the  
leakage while conducting In-Mast fuel assembly sipping during core       
off-load activities. The licensee sipped all 193 fuel assemblies and also
found a non-HEU fuel assembly, manufactured by Westinghouse, that had    
several leaking rods. The Westinghouse assembly had been in the reactor  
core for three operating cycles. The licensee removed the leaking LTA    
from the core, as well as the other three assemblies.                    
                                                                         
FCF performed ultrasonic testing on the LTAs to determine the extent of  
the problem and determined that only a single fuel rod in the LTA had    
been leaking. FCF was unable to observe the defect using fiber optic     
visual inspection. They did observe, however, that the defective fuel rod
had elongated by about 0.5 inch. FCF also performed high magnification   
visual testing to look for other indications of possible failure         
mechanisms such as bad welds, presence of foreign material, or other     
manufacturing defects. FCF is presently reviewing the LTA manufacturing  
records for evidence of a possible manufacturing error. Both TVA and FCF 
are in the process of attempting to determine the root cause of the      
defect and FCF will publish a report by the end of January 2001 with the 
results of its root cause analysis. Currently, TVA sees no correlation   
between the subject rod developing a leak and the fact that the rod      
contains HEU.                                                            
                                                                         
The staff has preliminarily concluded that there is no safety issue      
related to this failure. During the operating cycle, reactor coolant     
system radioactivity level remained below 50 percent of the Sequoyah     
Technical Specification limit for Dose Equivalent Iodine-131, which is   
0.35 microcuries per gram. TVA believes that this rod failure will have  
no long-term programmatic effect on the DOE plan to use HEU as reactor   
fuel in one or more TVA reactors, which may start as early as 2005.
