Subject: REACTOR SCRAM AND DECLARATION OF UNUSUAL EVENT
Discussion:
On October 2, at 6:25 a.m., Nine Mile Point Unit 1 (NMP1) was manually
scrammed in response to a stuck open reactor relief valve during reactor
startup and heatup. At the time of the scram, the reactor was operating
at less than one percent power, 35 psig and 273 F. All control rods
fully inserted and all safety systems functioned as expected. Subsequent
to the manual shutdown, a vacuum breaker in the tailpipe of the stuck
open relief valve failed, allowing steam to vent directly to the drywell
airspace. Condensation of the steam in the drywell and collection of the
leakage in the floor drain system exceeded 10 gallons per minute (GPM),
resulting in the operators declaring an Unusual Event at 7:12 a.m. The
Unusual Event was terminated at 9:05 a.m., following reactor cool down to
below 212 F and cessation of steam flow from the relief valve.
The licensee has initiated an event investigation team to evaluate the
relief valve and vacuum breaker problems. Initial inspections revealed a
sticking pilot valve on the relief valve and a broken disc stop and disc
stem on the vacuum breaker. The root cause for these failures will be
determined.
The resident inspectors have been monitoring licensee activities. A
region based inspector will conduct a special inspection to evaluate the
licensee's investigation team results and corrective actions.
Regional Action:
The licensee has contacted local media representatives and has issued two
press releases.

Subject: SEABROOK STATION ORGANIZATIONAL CHANGES
Discussion:
Seabrook Station announced today that Gene St. Pierre will become the
Station Director upon the retirement of Bill DiProfio on December 1,
2000. Gene St. Pierre has been employed at Seabrook Station since 1979
and is currently the Operations Manager. Brett Plummer, who was
previously the Operations Manager at Maine Yankee, will replace Gene as
the Operations Manager.
