The Salem licensee reported that two new General Electric (GE) Model
CR2940U301 switches failed preinstallation testing. This model switch
consists of a single normally-closed contact that is plunger-actuated and
contained in a plastic housing. It is used in the control circuit of GE
Magne-Blast circuit breakers as the spring-charging motor limit switch.
Failure of this switch will prevent the associated breaker from closing
on a demand signal. At Salem, this model switch is used on breakers in
safety-related and nonsafety-related applications. The licensee stated
that these failures appear to be caused by inadequate contact closure. GE
is performing a failure analysis, expected to be completed
February 29, 2000, which the licensee will incorporate into the final
reportability determination.
The licensee previously evaluated three other failures of this model
switch at Salem. This information will also be incorporated into the
final reportability determination.
Operational experience that may be related to this report is described in
NRC Information Notice 95-02, "Problems with General Electric CR2940
Contact Blocks in Medium-Voltage Circuit Breakers," dated January 17,
1995.

The Clinton licensee reported two test failures of a hydramotor (an
electrohydraulic push-type linear actuator). In the first case, the
redundant relief valve did not meet the fail-safe spring-return time test
acceptance criterion provided in the maintenance procedure. In the second
case, the hydramotor recycle rate was not consistent with maintenance
procedure criteria. In each case, the licensee had installed a new pump
assembly from stores into the hydramotor. When the licensee put the old
pump assemblies back, they tested satisfactorily. The new pump assemblies
are manufactured by ITT Barton, now Automatic Switch Company (ASCO). In
the first case, the part number is NS109761AQ and in the second case, the
part number is NS109761EQX07. In both cases, the licensee returned the
new pump assembly to ASCO for resolution.

Solidstate Controls, Inc., the vendor, supplied uninterruptible power
supplies and battery chargers, manufactured under its 10 CFR 50 Appendix
B program, to several nuclear power plants. The vendor reports a
potential defect in static switch control assemblies and regulated
rectifier control assemblies used in these components. The problem is
associated with a component unijunction transistor, part number
03-650007-00. In the uninterruptible power supplies, the static switch
sporadically changes from normal to bypass or alternate settings. In the
battery chargers, the regulated rectifier sporadically produces a higher
direct current voltage than desired.
The vendor notified the following affected nuclear power plants: Indian
Point 2, Catawba, Oconee, Arkansas, Waterford, Crystal River, Diablo
Canyon, Sequoyah, and Surry.
In subsequent telephone calls with the vendor, the NRC learned that some
of the affected customers have returned the spare parts to the vendor for
replacement with corrected assemblies.

The Nine Mile Unit 2 licensee reported nonconservative calculations for
environmental qualification of numerous components in multiple systems.
The supplier of the calculations, Stone and Webster Engineering
Corporation, assumed lowest expected ambient temperatures instead of
normal expected ambient temperatures for winter periods. The consequences
of correcting these assumptions is to reduce the qualified lifetimes for
about 30 components from more than 40 years to less than 40 years. For
several hundred components, the correction reduces qualified lifetimes
but not to less than 40 years.

Subject: Byron Station Management Changes
Discussion:
On March 10, 2000, Byron Station Site Vice President, Bill Levis,
announced the following management changes at Byron Station.
þ Steve Kuczynski will replace Mike Jurmain as the Byron Station
Maintenance Manager, who will be leaving Commonwealth Edison Company.
Steve Kuczynski was previously the Byron Station Nuclear Oversight
Manager.
þ Bob Deppi will be assuming the responsibilities of the Byron Station
Nuclear Oversight Manager. Bob Deppi was previously the Nuclear
Generation Group Operations Manager.
þ Marseyne Snow will be assuming the responsibilities of the Nuclear
Generation Group Operations Manager. Marseyne Snow was previously the
Byron Station Operations Manager.
þ Dave Hoots will be assuming the responsibilities of the Byron Station
Operations Manager. Dave Hoots was previously the Braidwood Station Shift
Operations Supervisor.
All personnel changes are expected to be completed by April 10, 2000.
