Subject: PALISADES SHUTDOWN FOR PLANNED MAINTENANCE OUTAGE-UPDATE ON PNO-
         III-00-003                                                      


Discussion:

This morning report is being issued to update the subject PNO. The steam 
generators and appropriate portions of the secondary plant were flushed  
to reduce elevated sodium concentrations to acceptable levels.           
Inspections of the A steam generator identified that an internal manway  
cover had come loose and was the cause of the water carryover problem.   
The manway cover and its associated clamp were modified and the cover was
reinstalled. Also, Primary Coolant Pump P-50B shaft seal was successfully
replaced. In addition, the alternate steam supply to the turbine driven  
auxiliary feedwater pump leak was appropriately addressed and the        
alternate steam supply line was subsequently isolated. An NOED was       
requested from and granted by NRR for operation with the alternate steam 
line out-of-service.                                                     
                                                                         
During subsequent plant startup activities, the licensee identified      
leakage from a control rod drive (CRD) mechanism seal assembly. The      
leakage was above what is normally seen in operations but well below     
Technical Specifications. The licensee placed the plant in cold shutdown 
to further investigate the leakage and to repair the CRD seal assembly.  
Licensee personnel identified that this CRD seal assembly and one other  
seal assembly, repaired earlier in the outage for leakage, were both part
of a group of seven assemblies with similar histories. Subsequently, a   
root cause investigation team was formed which included industry         
consultants. Currently, the licensee has disassembled all seven          
assemblies plus one additional assembly in an effort to identify a common
cause. As of February 24 no common cause has been identified.            

Regional Action:

Licensee activites are being monitored by the resident inspectors with   
assistance from regional specialists.
