Rotork Controls, a vendor of electric valve actuators, reported a        
potential for distortion of add-on moulded components, causing control   
switches to reset or fail to operate, in extreme circumstances. This     
applies to Type NA1 electric valve actuators fitted with an Add-on-Pak 1 
(AOP1) assembly, manufactured between 1978 and October 2001 and that have
a safety function and are used for end of travel indication. The         
circumstances for this potential distortion are (1) that the actuator is 
held in the fully open or closed condition and the AOP1 is set between   
zero 3.5-7.5 percent of end of travel, depending on the gear ratio, and  
(2) that the temperature of the AOP1 components is maintained above 80C  
(176F) for more than 10 minutes.

ASCO, a vendor of valve actuators, reported potential operational        
difficulties with certain pumps supplied in pump kits and intended for   
installation in NH95 Model B Hydramotor actuators. The vendor was        
notified by Framatome ANP, the Clinton licensee, and the Palo Verde      
licensee of excessive recycling of the pump, meaning actuation and       
de-actuation of the pump to maintain the powered position of the actuator
