General Electric(GE), a vendor of J-core molded case circuit breakers,   
reported an offset in instantaneous (magnetic) trip current when using   
lug and cable connections as compared to using bus bar connections for   
models TJJ, TJK, THJK, TB4 and TBC with both non-interchangeable and

Wyle Laboratories, a procurer of irradiation services from Georgia       
Institute of Technology, reported an error in irradiation dose units for 
all equipment irradiated with the cobalt-60 sources in the Georgia Tech  
hot cell from 7/24/1997 to 7/6/2001. Georgia Tech reported to Wyle       
Laboratories radiation doses in units of rads-air (absorbed dose in air) 
instead of Roentgens. Since one Roentgen is equal to 0.877 rad, the      
actual dose is less than the reported value.

General Electric, a vendor of CR105X auxiliary contact devices, reported 
a failure to change state caused by disengaged plunger arm and loose     
plunger post that is caused by a missing lock washer. These devices are  
sold safety related as a kit with convertible contacts (from normally    
open to normally closed) or as part of contactor assemblies (CR105 205   
and 305 series), starter assemblies (CR106, 206 and 306 series), and     
controller assemblies (CR109, 209 and 309 series) that may be part of    
7700 series motor control centers.                                       
                                                                         
The vendor states that CR105X kits contain the lock washer but the       
provided customer installation instructions are defective. The vendor    
states that its records do not indicate a high incidence of failure due  
to loosening of the plunger post and recommends inspection of installed  
CR1 05X auxiliary contacts at the next available outage and inspection of
spare CR105X auxiliary contacts before they are used.
