Subject: Pressurizer Code Safety Valve Failed to Lift During Testing     
Reportable Event Number: 38887                         

Discussion:

During testing, a Point Beach Unit 2 pressurizer code safety valve failed
to lift at the maximum test stand pressure of 2660 psig. The valve is a  
Crosby, Model HB-BP-86 Type E, self-actuating spring type safety valve.  
The vendor estimated that the actual lift pressure would have been 3530  
psig, 1045 psig above the nominal setpoint pressure of 2485 psig. The    
vendor determined that the cause of the setpoint change was due to the   
spindle being engaged in the disc holder located within the valve. Proper
assembly of the valve requires that the spindle be inserted past the     
threads of the disc holder until it turns freely. This was not performed 
during the jack-and-lap evolution and was determined by the vendor to be 
an isolated instance of human error.
