NRC Information Notice 2002-01: Metalclad Switchgear Failures and        
Consequent Losses of Offsite Power dated January 8, 2002

The Waterford licensee reported that pressurizer steam space sample      
inside and outside containment isolation valves would not close on       
demand. The licensee informed the valve vendor Flowserve Engineering of  
the galling and subsequent binding of the subject valves. The licensee   
states that on March 21, 2001, the vendor agreed that the valve failure  
is a Part 21 issue and that the vendor would issue the Part 21. The      
subject valves are 1/2 inch, 2500 Ib, WKM model 70-18-9 DRTS cage-guided 
globe valves.                                                            
                                                                         
On April 11, 2001, the vendor submitted its evaluation of the Waterford  
problem, stating that the margin to accommodate a slight misalignment of 
parts, contaminants, etc. was not adequate. Flowserve was modifying the  
design of the Model 70-18-9 valve and Model 70-19-9 valve to assure a    
minimum of .006" diametrical clearance between the cage and plug at      
operating temperature exceeding 4500F.                                   
                                                                         
The vendor notified the domestic nuclear power plant licensees that      
procured these valves from Flowserve: Peach Bottom, Waterford, Wolf      
Creek, and Crystal River.

The radiation monitor vendor Sorrento Electronics reported that an       
RM-2000 detector was not responding to changes in energization, showing  
constant radiation on the channel display. The affected channels would   
not return to a normal operating condition until the detector was        
reconnected. This condition affects safety-related gas, duct, and        
particulate iodine gas monitors at the Salem plant. The vendor states    
that it will notify individual purchasers of this problem and make a     
corrected version of the RM-2000 code available after November 1, 2001.

zero period acceleration value and the recently determined value for

The Vermont Yankee licensee reported blistering of paint primer in       
containment. The paint, Carboline Carbozine CS-11SG, was applied to the  
drywell shell (inner wall) to provide a moisture barrier at the          
shell-concrete interface. The paint vendor Carboline and an independent  
test laboratory evaluated the first part of the two-part paint as        
defective, probably because of a moisture problem during manufacture. The
vendor recommended application of a single coat of Carboline 890 as an   
acceptable interim corrective action until a permanent remedy is

The Terry steam turbine vendor Dresser-Rand reported a material          
misapplication in trip and throttle valves manufactured by Gimpel that   
were used on Terry turbines at nuclear power plants. On May 25, 2001, at 
Catawba Unit 2, the installed trip and throttle valve screw spindle      
failed. In 1992, at Palo Verde Unit 1, the licensee found that a similar 
installed screw spindle was cracked. The Catawba spindle was found to be 
American Iron and Steel Institute (AISI) Grade 1212 steel and the Palo   
Verde spindle was found to be AISI Grade 1213 steel. Dresser-Rand stated 
that Gimpel traced the supplied material to a single lot of about 50     
original screw spindles manufactured in 1974 and identified the          
originally affected trip and throttle valve serial numbers as:

Dresser-Rand recommends replacement of these screw spindles and that     
screw spindles manufactured in 1974 or 1975 be tested to confirm that the
