Subject: MANUAL REACTOR TRIP DUE TO STEAM GENERATOR LEVEL APPROACHING
LO-LO LEVEL SETPOINT
Discussion:
The licensee has completed troubleshooting activities and corrective
actions in response to the September 4, 2001, manual reactor trip.
Unit 1 achieved criticality at 12:39 a.m., on September 6, 2001.
The licensee's investigation determined that the loss of normal feedwater
flow to the #1 SG occurred when the #1 SG feedwater regulating valve shut
due to loss of a Vital AC Inverter. It was determined that an electronic
card failure caused the loss of output from the vital inverter.
The turbine driven AFW pump did start and inject feedwater into all SGs
during the event. The 1A motor driven auxiliary feedwater pump (MDAFW)
auto started, but did not provide feedwater flow to the SGs because the
pump discharge pressure control valve failed closed due to the loss of
power from the vital inverter. Plant operators secured the pump until
power was restored to the vital AC bus from a maintenance power supply.
Additionally, the 1B MDAFW pump did not automatically start, but was
promptly started by a control room operator. During subsequent
troubleshooting, the licensee physically inspected the most likely causes
of the failure of the 1B MDAFW pump to auto start. In addition the pump
was successfully stared twice using simulated automatic start signals.
The licensee also conducted a test which included initiation of a loss of
feedwater pumps actuation signal (same as for the event). All three AFW
pumps successfully started. Despite this review and testing, the
licensee was unable to determine the cause of the 1B MDAFW auto start
failure.
Reactor power level is currently at 20% and the unit is proceeding to
full power operations.

Subject: MANAGEMENT CHANGES
Discussion:
Duke Energy Corporation announced that Mr. Mike Glover (currently
Catawba's Operations Superintendent) will succeed Mr. Ron Jones as the
Station Manager at Catawba. As previously indicated in Morning Report
2-01-0014, dated August 16, 2001, Mr. Ron Jones will be assuming the
position of Station Manager at Oconee as of October 1, 2001. Mr.
Glover's successor has not been announced.
