Subject: MANUAL REACTOR TRIP DUE TO INCREASE IN MAIN CONDENSER PRESSURE  

Discussion:

At 5:28 p.m. on June 29, 2001, a manual reactor trip was initiated from  
100 percent power as required by procedures when main condenser          
backpressure limits had been reached.  The increased condenser           
backpressure occurred when cooling water flow was reduced because of an  
obstruction on the suction screens to the condenser cooling water (CCW)  
pumps.  The licensee determined that several fiberglass corrugated sheets
had broken loose and fallen into the cooling water tower basin and       
partially obstructed the suction screens to the CCW pumps. The sheet     
material was installed in the upper portion of the cooling water towers  
to reduce flow impingement forces on the cooling tower concrete basin.   
The licensee determined that the sheet material had been improperly      
installed.                                                               
                                                                         
The Unit is currently in Mode 5 (cold shutdown).  Subsequent to the Unit 
shutdown and independent of the reactor trip, while preparing to dump    
steam to the atmosphere via the steam generator pressure operated relief 
valves, a main steam isolation valve failed to completely close.  In     
addition, the licensee found several ice condenser doors partially open. 
The licensee is in the process of evaluating the cause and effecting     
repairs for these problems.                                              

Regional Action:

The residents are monitoring licensee repair and Unit restart activities.

Subject: TENNESSEE VALLEY AUTHORITY NUCLEAR (TVAN) ANNOUNCES MANAGEMENT  
         ROTOTIONAL ASSIGMENTS                                           


Discussion:

On June 29, 2001, TVAN announced several rotational assignments,         
effective immediately as follows:                                        
                                                                         
J. Bailey, Vice President of Engineering And Technical Services will lead
a special projects to consider the productive uses of the Bellefonte     
site. Jon Rupert, Chief Engineer, TVAN will act in Mr. Bailey's position.
                                                                         
                                                                         
J. Maddox, Manager, Engineering and Site Support at Watts Bar will serve 
as TVAN, Chief Engineer. J. Kammeyer will act in Mr. Maddox's position.
