NRC Information Notice 2000-17,Supplement 2: Crack in Weld Area of       
Reactor Coolant System Hot Leg Piping at V. C. Summer,

The U.S. Nuclear Regulatory Commission is issuing this information notice
(IN) supplement to provide updated information about the crack found in a
weld in the A loop hot leg pipe in the reactor coolant system (RCS) at

Subject: Part 21 - Internal Binding of the Terminal Shaft in Woodward EGB
         Hydraulic Actuators

The Summer licensee reported internal binding of a terminal shaft on a   
Woodward Type EGB hydraulic actuator unit on an emergency diesel         
generator (EDG). On 12 /21/00, after a full load surveillance run, the A 
EDG experienced load swings and frequency oscillations when the output   
breaker was opened. The EGA electronic control unit and EGB hydraulic    
actuator were replaced with components from the licensee's warehouse. A  
1.5 hour surveillance run yielded satisfactory results. A second 1.5 run 
was made to verify satisfactory performance. During the second run,      
moderate frequency oscillations were observed when the output breaker was
opened. The licensee installed monitoring instrumentation and performed a
third run. Severe frequency oscillations (from full fuel to no fuel)     
occurred on the third run. Even with the EGA control unit disconnected,  
the EGB actuator could not be stabilized. The licensee removed the EGB   
actuator and sent it to B.F. Goodrich/Fairbanks Morse (the EDG           
manufacturer) for evaluation.                                            
                                                                         
Subsequent investigation by B.F. Goodrich/Fairbanks Morse determined that
the binding was caused by clamping screws that were 1/8-inch longer than 
those specified by the governor vendor (Woodward). The licensee stated   
that the error was made during refurbishment of the EGB unit by Engine   
Systems, Incorporated (ESI), on 2/24/00. The licensee replaced the EGB   
units on both emergency diesel generators with units refurbished by B.F. 
Goodrich/Fairbanks Morse.
