Discussion:

On Friday, April 16, 2004, the first independent spent fuel storage installation (ISFSI) transfer of the current campaign was stopped during final fit-up of the Trans Nuclear shipping transfer cask to the Horizontal Storage Module (HSM) due to a fluid leak and loss of pressure in the hydraulic positioning system.

The transport vehicle was left in place on the ISFSI pad and appropriate security was maintained for the vehicle until repairs were made on Saturday, April 17, 2004. The licensee verified that the license and tech specs for the ISFSI were met throughout the transfer including the extended period with the HSM opened for receipt of fuel and with transfer cask loaded.

The licensee found that a hydraulic fitting associated with the transfer ram came into contact with a deck plate on the vehicle during fit-up, bending the fitting and causing a loss of hydraulic fluid. It appears that the length of the fitting was too long causing the impact with the deck plate. The last use of this vehicle was at Duane Arnold. The licensee's current assessment is that the fitting was replaced after the Duane Arnold use, prior to transfer to Oyster Creek.

After replacement of the broken parts on Saturday, April 17, 2004, the spent fuel was successfully placed in the HSM by 4:45 p.m.
