Discussion:

On May 7, 2006, during manufacturing, Sulzer Pumps, Inc. discovered weld repairs to two pump bowl castings provided by Stainless Steel Foundry and Engineering to be used in a replacement Essential Service Water Pump for the J. M. Farley Nuclear Power Plant.

The Sulzer Pumps procurement documents to the foundry required weld repairs to the castings be preceded by notification to Sulzer, submission for approval of both the proposed weld procedure (WPS) to be used, and the weld procedure qualification record (PQR). Weld maps were also required by their procurement documents.

Sulzer Pumps received material certifications from Stainless Steel Foundry and Engineering which certified no welding was performed to the Farley castings. However, during inspection processes, Sulzer discovered what appeared to be weld repairs. Subsequent investigation by Sulzer Pumps and foundry representatives concluded that undocumented and unapproved weld repairs were performed by the foundry.

Stainless Steel Foundry and Engineering previously provided impeller castings to Sulzer Pumps for orders that were shipped to Hatch and Indian Point. In light of the undocumented and unapproved weld repairs identified in the castings for the Farley order, the Hatch and Indian Point orders may also contain undocumented weld repairs.

All other castings from Stainless Steel Foundry and Engineering have been placed on hold and will be returned to the foundry.
