Subject: Federal Employees Health Benefits Program: Humana Medical Plan of Florida Service Area/Enrollment Code Error


This is to advise you that Humana Medical Plan of Florida codes (P7, P5, 7F, 9D, JH, EE) printed and distributed their 2000 Plan brochure (RI 73-278) with an error in a service area/enrollment code association.

Alachua, Bradford, Columbia, Dixie, Gilchrist, Levy, Putnam, and Union counties are listed in the brochure under code P5 (Jacksonville area), however, they SHOULD be associated with code 7F (Orlando/Gainesville area).

People who live or work in the Florida counties of Alachua, Bradford, Columbia, Dixie, Gilchrist, Levy, Putnam, or Union should select enrollment code 7F (which has a lower premium) - rather than code P5.

In contract year 1999, Alachua county was associated with P5. The other counties are new for 2000. Those currently enrolled in Alachua county should change to 7F and benefit from the premium savings.

Agencies are asked to advise their employees of this error.
