Directions: From Interstate 75, South take exit 217, or North use exit 217A. Travel West on State Road 70, approximately 2 miles. Peridia Office Park is located on the right just past 39th Street. Turn right into the office park then make first right. The clinic is the first building on the left.

Registration and Enrollment
To register or enroll for healthcare in person, you may do so at Bay Pines VAMC. If you wish to register by mail, you may contact Bay Pines VAMC or any Community Based Outpatient Clinic to receive a registration packet in the mail.
