The Office of Personnel Management has revised Standard Form (SF) 3106, Application for Refund of Retirement Deductions, Federal Employees Retirement System. This form is used by Federal employees applying for a refund of their retirement deductions. The new edition of the SF 3106, dated August 2000, is currently available. The only change that has been made to the form, is a revision in the Instructions For Completion of Your Application For Refund of Retirement Deductions section. We have added a statement that refunds of post-56 military service deposits permanently eliminate retirement rights for the period of military service covered by the deposit. This change is found in paragraph 2, under the heading Special Information for Applicant. The previous edition of the form, dated August 1998, is still usable. The SF 3106 includes a copy of the SF 3106A, Current/Former Spouse's Notification of Application for Refund of Retirement Deductions under the Federal Employees Retirement System. The SF 3106/SF 3106A can be ordered from the GSA Federal Supply Service using the normal FEDSTRIP/MILSTRIP procedures. Ordering information is provided below.
