We at Hurley chart vital signs only in certain situations. They are not used, for example in critical care. If the patient has an order for vital signs less frequently than q2h (so q 4h, or q day), those are entered. Flow sheets are used for more frequent vital signs, and resistance to removal of the paper form (mostly from physicians) is the reason most often given for their continued use. Pain scale, as a vital sign is charted, but it may only be once a shift.
