Subject: Meds Upon Admission/Discharge Planning
Does anyone collect "Medications Taken at Home" data upon a patient's
admission (Admission Database) and then use the data in the Discharge
Planning process? I would be interested in seeing how your pathways are
designed in both the admission process and the discharge process. Thanks.
