Oregon State employees who participate in a PEBB medical and/or dental plan are eligible to enroll their domestic partner. Domestic partners are also eligible to enroll in life insurance. Obtain a Domestic Partner Information Packet by printing the options below or by calling PEBB at (503) 373-1102 or (800) 788-0520 (outside Salem).
Employees who wish to enroll a domestic partner for PEBB benefits must complete and return the Affidavit of Domestic Partnership and the appropriate enrollment or update forms to their agency payroll, personnel or benefits office.
