Behavioral Health Code Sheet June 2005 [PDF-17KB]
This Job Aid includes Activity codes, Clinic codes, Type of Contact codes and other codes used when documenting a patient or administrative encounter. It is intended to be used as a guide or "cheat sheet" for the provider. The Activity codes listed in this code sheet are the most frequently used Activity codes only. A complete list of the Activity codes and their definitions can be found in Appendix A of the BHS v3.0 User Manual.

This Job Aid is a listing of BHS problem codes by category. It is intended to be used as a guide or "cheat sheet" for providers when doing data entry. It is not a complete list of the DSM-IV-TR diagnostic codes. The complete list of DSM-IV-TR and BHS problem codes and their definitions can be found in Appendix C of the BHS v3.0 User Manual.

BHS Export Error Messages [PDF-11KB]
This Job Aid is intended to assist in the process of exporting data from BHS v3.0 to IHS National Programs. A list of errors in data entry can be obtained before the export process by selecting “CHK” (Check Records Before Export) from the Export Utility Menu in the Managers Utility Module of BHS v3.0. This list of error codes also offers explanations and corrective action.
