Ordering Forms and Kits

The Oregon State Public Health Laboratory provides test collection kits and test request forms for tests performed at the OSPHL. There are two ways you can place your order:

1. Telephone the OSPHL Stockroom at 503-693-4114, or
   
2. Print the PDF file, complete the form, and mail it to the address indicated on each form.
