BOLI'S CIVIL RIGHTS COMPLAINT QUESTIONNAIRE
Important Information
Complaints filed with BOLI must be filed within a limited amount of time.

Complaints alleging unlawful discrimination must be filed within one (1) year of the date when the alleged discrimination occurred.

OSHA Note: Complaints alleging retaliation based upon reporting health or safety issues in the workplace must be filed within ninety (90) days of the occurrence of the retaliation.

This is only a first step: Once BOLI receives your questionnaire, you may be contacted to provide further information. When BOLI has determined that there is enough information to proceed, BOLI will either send you notice of your filed complaint or prepare a complaint and send that complaint to you for your approval and notarized signature.
 
You will not have officially filed a complaint until BOLI receives your signed complaint that contains the necessary elements of a prima facie case under BOLI’s jurisdiction.

Important: If your contact information changes at any time, you MUST notify BOLI of your updated information. If BOLI is unable to contact you, your questionnaire may not be processed, or your complaint may be dismissed.
