Question:I want to report patient abuse in a long-term care facility (nursing home, assisted living/boarding home or a adult family home. Who do I call?

Answer: You can report the abuse to the Long-Term Care Ombudsman Program at 1-800-562-6028. The Long-Term Care Ombudsman Program was established in 1978 through an amendment to the Older Americans Act of 1965.

Each state maintains an Ombudsman that serves as an advocate for residents of nursing homes, board and care homes, and assisted living. Ombudsmen provide inforomation about how to find a facility and what to do to get quality care. They are trained to resolve residence rights issues in long-term care facilities.
