At the 1999 Legislative Session, a new law was created forming the Public Safety Memorial Fund Board. This new law is in effect now and applies to family members of police officers (including reserves), corrections officers, fire service professionals (paid & volunteer), parole and probation officers, and youth correction officers. ORS 243.950 through 243.974 outlines potential benefits from the Memorial Fund to eligible family members, as follows: death benefits (up to $25,000), health and dental insurance benefits, educational scholarships, and mortgage benefits. Forms used by DPSST to administer the rules and laws of this legislation are available on this web site. Form M-1: application for benefits - to be completed by those applying for benefits; Form M-2: a designation of beneficiary form - to be completed by all public safety officers affected by this law; and kept on file at the employing agency (to be sent to DPSST with the form M-3); Form M-3: notice of death/disability - sent from the employing agency to DPSST (within three days) of the death or permanent total disability. This notifies the Public Safety Memorial Fund Board of an affected public safety officer’s death/disability. The employing agency must determine if the death/disability occurred while “in the line of duty”. Please refer to the rule for the definition. The Form M-3 is to be submitted to DPSST so the Memorial Fund Board may pay the beneficiary the death award. DPSST is asking the employing agency’s help by being responsible for providing a completed Form M-2 and M-3 within three days of death/disability so beneficiaries and family members do not need to wait needlessly for payment of benefits that may be due them. Employers may with to review an information checklist for use should an officer be killed or become disabled. Family members may wish to review the eligibility timeline for deadline requirements. The laws/rules are in effective now, but also apply to eligible family members of officers who died or became disabled as of January 1, 1997 (subject to availability of funds).
