Forms are updated from time to time, so rather than stockpiling them in your file cabinet until they are out of date, just download and print when needed. Click here for Investigative Guidelines. For reportable diseases lacking Oregon-specific investigative guidelines or case report forms, please contact the epidemiologist on call for assistance.

Please fax the first part of the form (demographic information) to ACD as soon as possible, even if you have not yet completed case investigation.
