2008 Commissioned Corps Pre-Retirement Seminar
The Commissioned Corps Personnel Office, CDC will host a Pre-Retirement Seminar for all USPHS Commissioned Corps Officers. The seminar is scheduled for April 24-25, 2008 in Atlanta, GA, Mr. Thomas Berry, Commissioned Corps Retirement Coordinator will facilitate the seminar. The seminar is open to all PHS officers; however, CDC/ATSDR officers will be given priority seating until March 21, 2008. All others will be selected based on the order their registration forms are received.
To register: Download, complete, and print the Pre-Retirement Seminar Registration form 
. Please fax the completed form to Mr. Aaron Arnold at 770-488-1970. Please be sure to include your email address. Your registration will be confirmed via email no later than March 28, 2008.
NO EMAILS OR ATTACHMENTS will be accepted – only faxed registration forms.
