There are 5 types of End-Stage Renal Disease facilities that are subject to certification: Renal Transplantation Center, Renal Dialysis Center, Renal Dialysis Facility, Self-Dialysis Unit, and Special Purpose (short-term) Renal Dialysis Facility.

Businesses must apply for facility license BEFORE accepting patients License: $165 yearly.
The Division, under the direction of the Health Care Financing Administration (HCFA), also determines eligibility of the facility for Medicare and Medicaid reimbursement certification. Contact HFD for further information is you wish your facility to be eligible.
