Mandatory Reporting Form (Form #7230 - Stock #300457)
Mandatory reports MUST be submitted on a DMV form. Physicians and health care providers must submit reports to DMV on the Mandatory Impairment Referral Form (see an example of the completed form for reference).
Voluntary Reporting Form (Form #6066 - Stock #300230)
Doctors, health care professionals, family members and social service providers may report an unsafe driver under DMV’s voluntary reporting system. These reports may be submitted on your agency/organization letterhead or on a Driver Evaluation Request Form.
Submitting Forms
Completed forms may be faxed to DMV’s Driver Safety Unit at (503) 945-5329, or mailed to:

Downloading/Printing/Ordering Forms
Forms may be downloaded and printed directly from the Internet, using the Save As and Print options from the File pull down menu when viewing a form in the section above.
Forms may be ordered in bulk from the ODOT Storeroom. To do so, fax a request on your agency/organization letterhead directly to the storeroom at (503) 986-2801.
Be sure to include the following in your request:
- Name of the form*
- Form number*
- Stock number*

Black and white copies of our brochures can be ordered in quantities of 50 from the ODOT Storeroom. To do so, fax a request on your agency/organization letterhead directly to the storeroom at (503) 986-2801.

Online Training |
|
An online presentation on DMV's Medically At-Risk Driver Program is now available. This audio and visual presentation is approximately 12 minutes in length.

Training Video |
|
Use the online order form to request copies of the Medically At-Risk Driver Training from DMV. The presentation is 18 minutes in length and available on VHS or DVD. The VHS or DVD (please specify your preference) will be mailed to you within five business days of your order.
