You may download tax forms from our Web site, http://www.oregon.gov/DOR/forms.shtml. If you would like us to mail forms to you, including prior-year forms and forms not available on the Internet, please e-mail us, forms.dor@state.or.us. Please include your phone number, name, and mailing address where you would like the forms sent. You may also order forms by calling our Tax Services Unit, 503-378-4988 (Salem and outside Oregon), or toll free from an Oregon prefix at 1-800-356-4222. You may also write to us for forms:
