2008 Public Health Action Plan Update: Celebrating Our First Five Years

Action Framework for a Comprehensive Public Health Strategy to Prevent Heart Disease and Stroke

The Action Framework above describes the public heath strategy to prevent heart disease and stroke.  The top portion shows a vision for the future—

  • Social and environmental conditions favorable to health lead to
  • Behavioral patterns that promote health; which leads to
  • Low population risk; which leads to
  • Few events/only rare deaths; which leads to
  • Full functional capacity/low risk of recurrence; which leads to
  • Good quality of life until death.

The next part of the framework describes the continuum of care.  On the "upstream" portion of the continuum it shows—

  • Policy and environmental change
  • Behavior change
  • Risk factor detection and control

On the "downstream" portion of the continuum it shows—

  • Emergency care/acute care management
  • Rehabilitation/long-term case management
  • End-of-life care

The next portion of the framework describes the intervention approaches and what the present reality is—

  • Unfavorable social and environmental conditions leads to
  • Adverse behavioral patterns; which leads to
  • Major risk factors; which leads to
  • First event/sudden death; which leads to
  • Disability/risk of recurrence; which leads to
  • Fatal cardiovascular disease (CVD) complications/decompensation.

Below that describes what the Healthy People 2010 goals are to increase the quality and years of healthy life and to eliminate disparities—

  • Goal 1: prevention of risk factors
  • Goal 2: detection and treatment of risk factors
  • Goal 3: early identification and treatment of heart attack/stroke
  • Goal 4: prevention of recurrent cardiovascular events
