Our office has been requested to provide a letter of recommendation for the below listed individuals to tour the Federal Law Enforcement Training Center (FLETC).   The proposed visit will be conducted on MONTH DAY, YEAR.

• Telephone Number of Organization/Agency where employed
• Facsimile Number of Organization/Agency where employed
• Email Address: (if available)
• Applicant poses no health risk to the United States:

• Applicant poses no security risk to the United States

Additional information that the FLETC will need is:

• Copy of each visitor’s passport information page only
• What country issued the visitor’s passport

• Purpose for the visit to FLETC
• Will the visitor’s bring interpreters with them?  If so, are they U.S. Citizens?  If not, we will need the same information on the interpreter as we do the visitor.
• Sponsoring agency name, point of contact (POC), POC email address and POC telephone number
