Application for Postgraduate Education Waiver (Only items highlighted in red shall be modified) NOTE: In order to be eligible to compete for a PG panel when an officer does not meet all of the eligibility requirements set forth in all PG guidance, a waiver request in the form of a CG memorandum must be submitted by the member through their respective Commanding Officer to CG PC (opm-1) and should address the following points using the template provided above:
1) Program applied for,
2) Justification for waiver, and
3) Acknowledgement of applicant responsibilities subsequent to waiver submission.
All waiver requests must be received by CG PC (opm-1) NLT the application deadline promulgated for the program. The preferred method of delivery is a scanned, signed, and endorsed copy delivered via email to LTJG G. Wayne Newton or YN1 Christopher Horn. Waivers may also be faxed to (202) 493-1618.

23 May 2008 - E-Resume submission for all programs except for Physician Assistant, MIT-SDM Fellowship, and the SLOAN Fellowship.

06 Jun 2008 - Command Endorsement submission for all programs except for Physician Assistant, MIT-SDM Fellowship, and the SLOAN Fellowship.

15 Jun 2008 - E-Resume and Command Endorsement submission for Physician Assistant.
