Below is an example of an ESO designation letter. The template needs to be updated with your command specific information (unit name, address, etc.).

Be sure to include the ESOs EMPLID as this is required for TACCTS account set up. Once your TACCTS account is established, you will receive an email notification with your ID and temporary password.

Once completed and signed, the letter can be faxed to: (405) 954-3684 or emailed as an attachment to CGI-PF-NRT_ESO_Emails@uscg.mil.
