Pacific Region Fires - 2003 (5 total) March 19, 2003 – Arguello Inc.
Remarks: The sales gas compressor had been out of service for maintenance. A short time after the operator brought the compressor back into service he noticed smoke coming from the turbine exhaust area. The operator activated the ESD and deluge systems. It was discovered that a portion of the insulation cover had been left off during maintenance operations April 30, 2003 – Nuevo Energy Company
Remarks: The rig crew started up the 500 KW generator that provides power to the workover rig and is located in an electrical room. Shortly thereafter, the driller smelled smoke, shut down the generator, and notified the lead operator on the platform. Both men inspected the electrical room but could not find the source of the odor. After about 30 minutes, the generator was restarted. A few minutes elapsed before the driller returned to the electrical room and saw flames emanating from under a control panel door. The driller immediately turned the generator off and the flames extinguished themselves. The fire was caused by deteriorated insulation on wiring which resulting in a short. The wiring was replaced. Remarks: The platform was in the midst of a planned electrical shutdown and was operating under diesel generator power when the generator motor stopped running. The generator was restarted. Three to five minutes later flames were observed coming from the electrical generation portion of the generator along with a strong electrical odor. The load was removed from the generator and the engine died. The fire was extinguished using a handheld halon fire extinguisher. July 29, 2003 – Exxon Mobil Corporation
Remarks: A small fire occurred in the 4th floor laundry room when a 220 volt plug went to ground and blew a hole through the junction box causing a flash fire. The fire spread up the wall and into the ceiling area due to high levels of lint within the wall space and dropped ceiling. Flames developed in the ceiling area by the exhaust air conditioning vent where there was enough fuel to sustain the fire. The fire was extinguished using two 10 lb. fire extinguishers. The root cause was the junction box and wiring explosions, due to a loose connection and heat generation, which resulted in the failure of the insulation and the short to ground. Remarks: A small smoldering fire occurred when hot slag from a gas pipeline welding operation fell onto a rubber mat covering equipment in the work area. Smoldering was observed by the firewatch who immediately doused the area with water from a charged fire hose. No injuries, property damage, or platform shutdown occurred due to this incident.
