Colorado Department of Public Health & Environment Laboratory Services Division 8100 Lowry Blvd. Denver , CO   80230 (303)692-3090	Clinical Laboratories in Colorado are registered by the State Certification Program under the federal regulation known as the Clinical Laboratory Improvement Amendments (CLIA). The regulation requires certification of all sites conducting any clinical test including those considered simple and without regard for payment to comply. Link:  http://www.cdphe.state.co.us/lr/Certification/cr_hom.asp Medicare and Medicaid laboratory reimbursements are denied when appropriate certificates do not exist. Laboratories that conduct complex testing are inspected every two years. Details on the federal CLIA regulations are available on the U.S. FDA's web site. Link:  http://www.fda.gov/CDRH/clia/ Dairy laboratories are evaluated by Certification Program staff to assure compliance with the federal Food and Drug Administration (FDA) Pasteurized Milk Ordinance. Colorado Division of Registrations Board of Dental Examiners 1560 Broadway #1310 DENVER , CO   80202 (303)894-7758 Fax: (303)894-7764	A dental lab must have a work order in its possession from a licensed dentist to perform any construction, alteration, repair or duplication of any denture, plate, partial, bridge, splint, orthodontic or prosthetic appliance. Dental labs cannot work directly with the public. For more information contact the State Board of Dental Examiners. (12-35-130, C.R.S.)
