TO OBTAIN A PARKING PLACARD:
- PRINT and complete all applicable sections of the application form . A separate instruction sheet is provided with the application.
- Obtain your doctor's certification as designated on the form.
- Submit the application form to the nearest County Issuing Agency noted on the form.
- Renewals may be done by mail.

Remember . . .
- The only legal use of a placard is by the person to whom it is issued. The disabled person does not have to own or drive the vehicle to use the placard.
- Placard abuse or misuse can result in the suspension or revocation of the placard and loss of the privileges it provides. Placard abuse is punishable by a fine of not less than $250, or not more than $500.
- Hawaii law requires placard holders to carry a valid State of Hawaii Parking for Persons with Disabilities identification card whenever placards are in use.
- Diagnosed with an arthritic, neurological, renal, orthopedic or oncological condition.
- Use or need assistance from a brace, cane, crutch, or another person, prosthetic device, wheelchair, or other assistive device.
- Restricted by lung disease.
- Use portable oxygen.
- Restricted by a cardiovascular disease.
Temporary parking placards are issued to a qualifying person who has a temporary disability which lasts for a period of not more than six months. A fee is charged to obtain a temporary placard.
Replacement placards are issued if you have a lost, stolen, or mutilated four-year or temporary placard, and are applying for a replacement placard. A fee is charged to obtain a replacement placard.
Placards for persons with disabilities from other states are recognized in Hawaii. You must bring the original placard. Photocopies are not accepted.
